Obtain the permalink for an item record in the OCLC Worldcat database. Click on the permalink icon that appears in the upper right-hand corner of every item record.  Copy and paste the link from the box that pops up into the above field. 

Enter the the title of the material you are requesting.

Enter your name into the above field.

Enter your 14-digit library card number into the above field.  The library card number appears on the bar code on the back of your library card.  Example: 20022000123456.
